Trellus takes a precision medicine approach to treating patients with chronic illness and ensures they get the right interventions at the right time, with the right people.
In the US, 1.3% of adults have been diagnosed with IBD (either ulcerative colitis or Crohn’s disease). This common chronic condition can be all-consuming for the patient, both because of dealing with the symptoms and navigating through their healthcare needs.
To address the strain of having multiple medical care teams, Trellus Health is taking a resilience-driven approach to the problem and empowering patients to take control of their health. Their platform provides a multidisciplinary care team that includes mental health providers, nurses, dieticians, and doctors. The goal is precise utilisation of the health and care system for a personalised care plan for each person.
Patients have 24/7 access to digital tools for remotely monitoring their health, self-care, nutrition, and stress-relief. The care team helps the patient strengthen resilience with one-on-one coaching, dependent on what the patient needs at that moment. It may be mental health support, medication monitoring, or self-care enhancement.
The platform is designed to not only monitor the patient’s physical health but to help them manage life with a chronic illness and enable them to excel in their life. Though IBD is their main focus at the moment, the company plans to expand and include other chronic diseases.
